Chinese Economic Reform Two years after the death of Mao Zedong in 1976, it became apparent to many of China's leaders that economic reform was necessary. During his tenure as China's premier, Mao had encouraged social movements such as the Great Leap Forward and the Cultural Revolution which had had as their bases ideologies such as serving the people and maintaining the class struggle. By 1978 "Chinese leaders were searching for a solution to serious economic problems produced by Hua Guofeng, the man who had succeeded Mao Zedong as CCP leader after Mao's death" (Shirk 35). Hua had demonstrated a desire to continue the ideologically based movements of Mao. Unfortunately, these movements had left China in a state where "agriculture was stagnant, industrial production was low, and the people's living standards had not increased in twenty years" (Nathan 200). This last area was particularly troubling. While "the gross output value of industry and agriculture increased by 810 percent and national income grew by 420 percent [between 1952 and 1980] ... average individual income increased by only 100 percent" (Ma Hong quoted in Shirk 28). However, attempts at economic reform in China were introduced not only due to some kind of generosity on the part of the Chinese Communist Party to increase the populace's living standards. It had become clear to members of the CCP that economic reform would fulfill a political purpose as well since the party felt, properly it would seem, that it had suffered a loss of support. As Susan L. Shirk describes the situation in The Political Logic of Economic Reform in China, restoring the CCP's prestige required improving economic performance and raising living standards. The traumatic experience of the Cultural Revolution had eroded popular trust in the moral and political virtue of the CCP. The party's leaders decided to shift the base of party legitimacy from virtue to competence, and to do that they had to demonstrate that they could deliver the goods. (23) This movement "from virtue to competence" seemed to mark a serious departure from orthodox Chinese political theory. Confucius himself had posited in the fifth century BCE that those individuals who best demonstrated what he referred to as moral force should lead the nation. Using this principle as a guide, China had for centuries attempted to choose at least its bureaucratic leaders by administering a test to determine their moral force. After the Communist takeover of the country, Mao continued this emphasis on moral force by demanding that Chinese citizens demonstrate what he referred to as "correct consciousness." This correct consciousness could be exhibited, Mao believed, by the way people lived. Needless to say, that which constituted correct consciousness was often determined and assessed by Mao. Nevertheless, the ideal of moral force was still a potent one in China even after the Communist takeover. It is noteworthy that Shirk feels that the Chinese Communist Party leaders saw economic reform as a way to regain their and their party's moral virtue even after Mao's death. Thus, paradoxically, by demonstrating their expertise in a more practical area of competence, the leaders of the CCP felt they could demonstrate how they were serving the people. To be sure, the move toward economic reform came about as a result of a "changed domestic and international environment, which altered the leadership's perception of the factors that affect China's national security and social stability" (Xu 247). But Shirk feels that, in those pre-Tienenmen days, such a move came about also as a result of an attempt by CCP leaders to demonstrate, in a more practical and thus less obviously ideological manner than Mao had done, their moral force. This is not to say that the idea of economic reform was embraced enthusiastically by all members of the leadership of the Chinese Communist Party in 1978. To a great extent, the issue of economic reform became politicized as the issue was used as a means by Deng Xiaoping to attain the leadership of the Chinese Communist Party. Mao's successor, Hua Guofeng, had "tried to prove himself a worthy successor to Mao by draping himself in the mantle of Maoist tradition. His approach to economic development was orthodox Maoism with an up-to-date, international twist" (Shirk 35). This approach was tied heavily to the development of China's oil reserves. "[W]hen [in 1978] estimates of the oil reserves were revised downward[,] commitments to import plants and expand heavy industry could not be sustained" (Shirk 35). I had thereafter no desire to tear down that veil, to creep through; I held all beyond it in common contempt and lived above it†¦.† (Du Bois, Chapter 1). But he goes on to say that even though he in h is own vision had decided to keep all beyond it in contempt and beat them all, he realized that he could not, for everything was theirs to own, and he could merely desire to own it from them. Du Bois further remarked at how it felt to be an outcast, or a 'problem', or to be viewed as one, and how as a result of it, he found his peers withdrawing to within this Veil and wasting themselves away in bitterness. Du Bois also says that the reason racism exists and continue to block the inhibition of the African-American man is because of this Veil and the distance and isolation they feel beneath it, a feeling, that despite the democracy and progress is still applicable to and experienced by many (Alexander, 2008). There were not too many factors that contributed to the group’s problems, but if I had to name one I would probably say that we were too relaxed at times. It was good that we all got on well as a group, but sometimes this togetherness would lead to going off topic because we had no one to keep us on track. I would not change the fact that we were friendly with each other, but there could have been a leader appointed at the beginning so that there was someone to keep us all in check. The first time I met Trish I thought that she was a time-orientated person. She just had a look on her face that showed determination and focus. My impression of Trish has stayed relatively the same because for our group she was the one to organize everything even though there was not a group leader. For Paul, I did not know him that well but I saw him as someone who could be useful as a conversation starter. Espousing the notion that some ideas have lasted over centuries and are as relevant today as when they were first conceived, Perennialism urges that these ideas should be the focus of education. According to Perennialists, when students are immersed in the study of those profound and enduring ideas, they will appreciate learning for its own sake and become true intellectuals. For Perennialists, the aim of education is to ensure that students acquire understandings about the great ideas of Western civilization. These ideas have the potential for solving problems in any era. The focus is to teach ideas that are everlasting, to seek enduring truths which are constant, not changing, as the natural and human worlds at their most essential level, do not change. A Perennialist views nature, human nature, and the underlying principles of existence as constant. The principles of knowledge are enduring. The study of philosophy is thus a crucial part of the Perennialist curriculum. Perennialists regard essentialism, and its view that knowledge stems primarily from the empirical findings of scientists, as undermining the importance of our capacity to reason as individuals; that is, to think deeply, analytically, flexibly, and imaginatively. Perennialists emphasize, though, that students should not be taught information that may soon be obsolete or found to be incorrect because of future scientific and technological findings. Perennialists criticize the vast amount of discrete factual information that educators traditionally have required students to absorb. Perennialists urge schools to spend more time teaching about concepts and explaining how these concepts are meaningful to students. Particularly at the high school and university levels, Perennialists decry undue reliance on textbooks and lectures to communicate ideas. Perennialists suggest that a greater emphasis be placed on teacher-guided seminars, where students and teachers engage in Socratic dialogues, or mutual inquiry sessions, to develop an enhanced understanding of historys most timeless concepts. In addition, Perennialists recommend that students learn directly from reading and analyzing the Great Books. These are the creative works by historys finest thinkers and writers, which Perennialists believe are as profound, beautiful, and meaningful today as when they were written. Perennialists lament the change in universities over the centuries from places where students (and teachers) pursued truth for its own sake to mere glorified training grounds for the students careers. They believe that there is a common core of knowledge that needs to be transmitted to students in a systematic, disciplined way. The emphasis in this conservative perspective is on intellectual and moral standards that schools should teach. The core of the curriculum is essential knowledge and skills and academic rigor. Schooling should be practical, preparing students to become valuable members of society. It should focus on facts-the objective reality out thereand the basics, training students to read, write, speak, and compute clearly and logically. Schools should not try to set or influence policies. A Perennialist asks, What are the essential things a student must know? Within a Perennialist curriculum, activities are tied to the past and not to what is essential right now. The Perennialist values the past but he or she likes the back to basics movement inasmuch as the basics are the learnings of the great minds of the past. Students should be taught hard work, respect for authority, and discipline. Teachers are to help students keep their non-productive instincts in check, such as aggression or mindlessness. For the Perennialist, a great education is a study of the classics. A Perennialist teacher holds fast to a constant curriculum and holds fast to concrete materials that define what he or she will teach (rather than assessing the class and meeting the students needs). A great proponent of Perennialism is Mortimer Adler, Professor of Philosophy at Columbia University. He wrote the Paidea Proposal. His basic tenant is that an individual learns best by studying the classics. The Paideia program seeks to establish a course of study that is general, not specialized; liberal, not vocational; humanistic, not technical. Only in this way can it fulfill the meaning of the words paideia and humanities, which signify the general learning that should be in the possession of every human being. The child should be taught certain basic subjects that will acquaint him/her with the worlds permanencies: English, languages, history, mathematics, natural sciences, the fine arts, and philosophy. The child should learn reading and writing, how to speak and how to listen. He is a social being and lives in a community of men. Thus he must use his reason his powers of rationality to communicate with other men. The three Rs ARE important. Perennialists hold that subject matter should be the center of the curriculum. They feel that greater educational opportunities have lowered standards in the schools. They are particularly critical of the progressives who have placed emphasis on the interests of the child and teaching through activity and freedom. Education should introduce the pupil to the universal concerns of mankind thorough the study of the great works of literature, philosophy, history, and science. Such works represent mans finest insights into his own nature. The insights presented in these works are eternal. They do not change; they are never dated. In this way, s/he learns truths that are more important than any s/he could find by dipping into this or that in the contemporary scene, or by pursing his own interests. Education requires hard work and effort. If the student is interested in the subject, well and good, but if not, s/he still must be required to do the tasks. After all, the student does not know what s/he will need in the future. Motivation is fine, and if the teacher can provide it, well and good, but if not, the student must work at the task regardless. Discipline in the classroom is essential to learning. A noisy classroom is not a desirable place for learning to take place. Order is vital. Students cannot always think of the future. Consequently, teachers and parents must help students overcome their desire for immediate ends and direct them to future objectives. Self-discipline is fine, but most of the times it is insufficient. Adults therefore must give direction. The classroom is teacher-centered. The teacher as a professional is far more likely to know what her students will need to know in the future than do they. Study is hard work. Concentration and effort is required. Students must therefore being culturated in the process. Essentialists aim to instill students with the essentials of academic knowledge, patriotism, and character development. This traditional approach is meant to train the mind, promote reasoning, and ensure a common culture. Essentialism tries to instill all students with the most essential or basic academic knowledge and skills and character development. Essentialists believe that teachers should try to embed traditional moral values and virtues such as respect for authority, perseverance, fidelity to duty, consideration for others, and practicality and intellectual knowledge that students need to become model citizens. The foundation of essentialist curriculum is based on traditional disciplines such as math, natural science, history, foreign language, and literature. Essentialists frown upon vocational courses. In the essentialist system, students are required to master a set body of information and basic techniques for their grade level before they are promoted to the next higher grade. The content gradually moves towards more complex skills and detailed knowledge. Essentialists argue that classrooms should be teacher-oriented. The teacher should serve as an intellectual and moral role model for the students. The teachers or administrators decide what is most important for the students to learn with little regard to the student interests. The teachers also focus on achievement test scores as a means of evaluating progress. The essentialist classroom is centered on students being taught about the people, events, ideas, and institutions that have shaped American society. Essentialists hope that when students leave school, they will not only possess basic knowledge and skills, but they will also have disciplined, practical minds, capable of applying lessons learned in school in the real world. Essentialism is different from what Dewey would like to see in the schools. Students in this system would sit in rows and be taught in masses. The students would learn passively by sitting in their desks and listening to the teacher. An example of essentialism would be lecture based introduction classes taught at universities. After they have completed this course, they will take the next level course and apply what they have learned previously. English 101 and English 102 are a specific example of essentialism.

Don't be mistaken by the word D-day it did not all happens in just one day but many days. D-day was just a code name for the day that Operation Overload started. D-day is very well known for the beginning of the end of the war in Europe and Hitler's rule over most of the ruined continent of Europe. Marbury v. Madison Essays - 6th United States Congress, Free Essays Marbury v. Madison Constitutional Law Marbury v. Madison Marbury v. Madison, one of the first Supreme Court cases asserting the power of judicial review, is an effective argument for this power; however, it lacks direct textual basis for the decision. Marshall managed to get away with this deficiency because of the silence on many issues and the vague wording of the Constitution. During the early testing period when few precedents existed, there was much debate about fundamental issues concerning what was intended by the words of the Constitution and which part of government should have the final word in defining the meaning of these words. Marshall used the Marbury case to establish the Supreme Court's place as the final judge. Marshall identified three major questions that needed to be answered before the Court could rule on the Marbury v. Madison case. The first of these was, "Has the applicant a right to the commission he demands?" The Constitution allows that "the Congress may by Law vest the Appointment of such inferior Officers, as they think proper, in the President alone, . . . " (Art. II, 2). The Judiciary Act of 1793 had given the President the right to appoint federal judges and justices of the peace; there is no dispute that such an appointment was within the scope of the president's powers. Debate arises because the Constitution is silent on the exact time at which the appointment is considered complete. The Supreme Court ruled that "when a commission has been signed by the president, the appointment is made; and that the commission is complete, when the seal of the United States has been affixed to it by the [secretary of state]." This ruling does not have direct constitutional support, but it is not an unreasonable decision. The second question which Marshall addressed was, "If [Marbury] has a right, and that right has been violated, do the laws of this country afford him a remedy?" The answer is logically yes although there are no specific words in the Constitution to support such an answer. Based on the type of government intended by the Constitution, the government is expected to protect individual liberty. As Marshall says, "[The government] will certainly cease to deserve [to be termed a government of laws, and not of men] if the laws furnish no remedy for the violation of a vested right." However, with this assertion Marshall established the power of the Supreme Court to review actions of the executive branch - a power that does not stem directly from the Constitution. The third and final question which Marshall addressed was whether Marbury "is entitled to the remedy for which he applies." Marshall further divides this question into two parts: the nature of the writ and the power of the Supreme Court. In examining the nature of the writ, Marshall solidifies further the Supreme Court authority over members of the executive branch. Marshall admits that "the officer to whom [the writ] is to be directed, must be one to whom, on legal principles, such writ may be directed . . . " and that the Supreme Court cannot "enquire how the executive, or executive officers, perform duties in which they have discretion." Yet Marshall insists that the Supreme Court can issue a mandamus "[where the head of a department] is directed by law to do a certain act affecting the absolute rights of individuals." This assertion does not have Constitutional basis. The Constitution does not expressly grant the Supreme Court power over either of the other branches of government. Finally Marshall gets to the question based on which he decides the case - the Supreme Court's jurisdiction over this case. For the first time in this case, Marshall uses direct constitutional basis to make his ruling. He argues that, "If it had been intended to leave it in the discretion of the legislature to apportion the judicial power between the supreme and inferior courts according to the will of that body, it would certainly have been useless to have proceeded further than to have defined the judicial power . . . The plain import of the words seems to be, that in one class of cases its jurisdiction is original and not appellate; in the other it is appellate, and not original." He bases this ruling on Art. III 2, which enumerates the cases in which the Supreme Court shall have original jurisdiction. Marshall further maintains that the Constitution is the supreme law of the land. How PG Brought the Diaper Revolution to China Essay When Procter Gamble set out to sell Pampers in China more than a decade ago, it faced a daunting marketing challenge: PG didn’t just have to persuade parents that its diapers were the best. It had to persuade many of them that they needed diapers at all. The disposable diaper — a throwaway commodity in the West — just wasn’t part of the cultural norm in the Chinese nursery. Babies wore cloth diapers, or in many cases, no diaper at all. And that, says Bruce Brown, who’s in charge of PG’s $2 billion RD budget, is why China presented — and still presents — such a huge opportunity. Today, after years of exhaustive research and plenty of missteps, Pampers is the No. 1-selling diaper in China and the company, in many ways, is just getting started there. The diaper market in China is booming. It stands at $1. 4 billion — roughly a quarter the size of the U. S. market — and is projected to grow 40 percent over the next few years, according to research firm Datamonitor. PG’s success in China has helped CEO Bob McDonald set some bold goals. Last October, he laid out a plan to add one billion customers over the next five years by promoting PG brands throughout some of the poorest corners of the world. How will PG go about doing that? To get a sense, just look at the way it cracked — and to a large degree created — the market for disposable diapers in China. Learning From Failure When PG first launched Pampers in China in 1998, the effort flopped. Instead of developing a unique product for the market, PG made a lower-quality version of U. S. and European diapers, wrongly assuming that parents would buy them if they were cheap enough. â€Å"It just didn’t work,† Brown says. Chinese split-pants, or kaidangku. There, potty training often begins as early as six months, and children wear what’s called kaidangku — colorful open-crotch pants that let them squat and relieve themselves in open areas. Pampers’ pitch wasn’t compelling people to try something new — and neither was the product itself. â€Å"We scrimped on the softness in the earlier versions,† says Kelly Anchrum, director of global baby care, external relations, and sustainability. â€Å"It had a more plasticky feel. It took us awhile to figure out that softness was just as important to moms in a developing market. † PG had tried a similarly watered-down approach earlier in the decade, when it launched laundry and hair-care brands in several emerging markets. Those products also failed, Brown says. After these experiences, the company in 2001 came up with a new approach to product development: â€Å"Delight, don’t dilute. † In other words, the diaper needed to be cheap, but it also had to do what other cheap diapers didn’t — keep a baby dry for 10 hours and be as comfortable as cloth. So PG added softness, dialed down the plastic feel, and increased the absorption capability of the diaper. To bring down the cost, the company developed more efficient technology platforms and moved manufacturing operations to China to eliminate shipping costs. The revamped diaper, Pampers Cloth Like Dry, hit retail shelves in China’s largest cities in 2006, selling for the equivalent of 10 cents in local currency, less than half the cost of a Pampers diaper in the United States. The Universal Pitch PG had the right diaper and the right price point. Now it faced the bigger challenge. â€Å"You have to convince someone that they need this thing,† says Ali Dibadj, an analyst who covers PG at Sanford C. Bernstein Co. For Frances Roberts, global brand franchise leader for Pampers, every trip to China was (and still is) an opportunity to learn more about Chinese nursery habits. It’s part of the PG ethos that brand leaders visit consumers in their own homes — something Roberts has done in dozens of countries, including Germany, Russia, and Jakarta. The goal is to uncover the nuances of each market, and early on in its diaper research PG discovered a universal need. â€Å"Moms say the same things over and over,† Roberts says. Their cry: We want more sleep. With the help of the Beijing Children’s Hospital’s Sleep Research Center, PG researchers conducted two exhaustive studies between 2005 and 2006, involving 6,800 home visits, and more than 1,000 babies throughout eight cities in China. Instead of cloth, the research subjects were tucked into bed with Pampers. The results: PG reported that the babies who wore the disposables fell asleep 30 percent faster and slept an extra 30 minutes every night. The study even linked the extra sleep to improved cognitive development, a compelling point in a society obsessed with academic achievement. PG then put its marketing machine into motion. Pampers launched the â€Å"Golden Sleep† campaign in 2007, which included mass carnivals and in-store campaigns in China’s biggest urban areas. A viral campaign on the Pampers Chinese web site asked parents to upload photos of their sleeping babies to drive home the study’s sleep message. The response was impressive: 200,000 photos, which PG used to create a 660-square-meter photomontage at a retail store in Shanghai. The ad campaign boasted â€Å"scientific† results, such as â€Å"Baby Sleeps with 50% Less Disruption† and â€Å"Baby Falls Asleep 30% Faster. No diaper brand, not even rival Kimberly-Clark, maker of Huggies, has come close to spending as much on advertising in China, according to CTR Market Research, the China-based division of American media researcher TNS Media Intelligence. Since 2006, Pampers’ measured media spend topped 3. 2 billion yuan, or about $476 million — more than three times as much as any other brand. In 2009 alone, PG spent $69 million, compared to Kimberly-Clark’s $12 million spend for Huggies. Ruling the Nursery — in China and Around the World Today, Pampers is the top-selling brand in China, a country where about a decade ago the disposable diaper category hardly existed. PG does not release sales figures for specific countries, but Datamonitor estimates that the company has captured more than 30 percent of the $1. 4 billion market. Karl Gerth, an Oxford professor who researches the spread of consumerism in China, says PG’s marketing campaigns strike the right tone. â€Å"You don’t want to come off as paternalistic,† says Gerth, who wrote the book â€Å"China Made: Consumer Culture and the Creation of the Nation. â€Å"The idea that Pampers brings a scientific backing and gives children an edge in their environment — that’s a brilliant way to stand out from the competition. † You could argue that it’s easy being No. 1 when the market is still small. But PG still has a lot of work to do. The company faces challenges from private-label and domestic brands, including the No. 2 market leader, Hengan International Group, which has steadily grown its market share to 20 percent. Local brands, meantime, are catching up with better products, marketing, and distribution. Chinese consumers are going to want to root for the home team,† Gerth says. And there’s still the challenge of making disposables a habit. On average, diaper use still amounts to less than one a day. â€Å"We’ve only just begun to scratch the surface [in China],† Dimitri Panayotopoulos, vice chairman of global household care, told investors in a 2008 analyst meeting. There’s even bigger potential in India, where the birth rate is almost double that of China but the diaper market remains tiny at about $43. 4 million. (Pampers is the top-selling brand there, too. So now, PG plans to take the sleep argument throughout rural and poor areas in India and elsewhere. The company also makes its case by positioning itself as a baby-care educator. Pampers sponsors healthcare-outreach programs such as a rural immunization program in China and mobile medical-care vans in Pakistan and Morocco. In India, there’s a door-to-door program that offers baby-care tips and diaper samples for moms. Of course, PG tweaks the sales pitch to fit different markets; that’s what the company is known for. In India, for instance, the convenience of disposable diapers doesn’t resonate with parents. The company’s consumer research found that many Indian mothers think that only lazy moms put their babies in disposable diapers that last a full night. Comparison of unusual administration methods of morphine Morphine is a widely used analgesic. Its clinical use and the high dependency factor (morphine and heroin have proven to be the most addictive opiates) brought up the need to investigate the classic routes of administration (oral, rectal, iv, sc) and compare them with some not so well established routes (nebulised, intrathecal, transdermal, sublingual etc). Introduction Morphine is a strong opioid derived from the opium poppy, Papaver somniferum1. It is used for the management of moderate and severe pain. It acts directly to central nervous system exhibiting a quick time of action. Along with pain relief, it causes the sense of well being (euphoria). It can also produce a series of other central and peripheral effects such as sedation, cough suppression, nausea, constipation and might cause histamine release2. It is used during surgeries for anaesthesia and as a pain reliever after the operation. It is a drug of choice for terminal care and it is really useful for pain management in cancer patients3. Morphine is available at several formulations and different drug administration routes. The selection of the appropriate administration route is dependent on the extent of pain control required. For instance, parenteral morphine is used for acute severe pain, while oral morphine is used for the treatment of temporary pain. The location of the pain will also determine if it is necessary to apply topical preparations or not. Patients preference (or fear) for a specific drug route and other psychosocial factors might also affect the choice of the formulation and patients compliance and finally medicines effectiveness. Discussion Oral route Regarding oral route, morphine comes as a solution and as immediate or controlled (systained) release tablets and capsules(fig 2). Fig 2. Oral formulations of morphine Oral Suspension Morphine solutions come in different strengths. Oral solutions can be prescribed by writing the formula Morphine HCl 5mg and Chloroform water to 5ml but the proportion can be altered. Morphine Sulphate solutions (Oramorph) are available at 10mg/5ml and as concentrated solutions of 100mg/5ml3. Other excipients include: ethanol (96%- morphine is slightly soluble in ethanol), corn syrup, sucrose, methyl parahydroxybenzoate (E218), propyl parahydroxybenzoate (E216) and purified water. Due to the high content of alcohol this drug is not recommended to alcohol misusers. Normal adult dose is 10-20mg every four hours although when prescribed in palliative care dosage varies higher or lower according to the severity of the pain and kidney function respectively3. As this formulation is not readily absorbed from GI tract, in order to produce same effects as iv dose a 50%-100% increase in dose must be considered. Overdose risk is high and can cause hypotension, respiratory depression and in some cases death. Naloxone which is an opiate antagonist is the first line treatment in case of overdosing5. Tablets and Capsules Solid dosage forms contain morphine in form of salt e.g Morphine Sulphate and Morphine hydrochlorate. Theses salts in the body will be available as morphine bases. Tablets are available in two preparations immediate release and modified release. The short acting tablets come in 10mg, 20mg and 50mg3. They are given every 2-4 hours to control and stabilise breakthrough pain. These tablets are a way of determining the amount of morphine needed to manage pain. Once the quantity of morphine is established the patient can take slow-release tablets and capsules. These formulations contain enough morphine to control pain for 12 or 24 hours. The starting dose varies and from 10-20mg every 12hours if the patient has not taken any other painkiller previously, to 20-30 mg every 12 hours in cases of replacement weak analgesics3. Note only the dose can increase whereas the frequency must remain the same i.e. every 12 hours. It can take up to 48 hours for the morphine to reach the required plasma levels meaning that it is important to be taken regularly otherwise they lose their effectiveness5. If the patience experience pain between doses an additional dose must be given and the GP should decide if it is necessary to increase the daily dose. Oral morphine is a well established drug delivery method however; problems such as the first pass metabolism, the slow onset and the risk of overdose have raised questions on whether other novel administration routes can be used. Intravenous This route is used only in hospitals and in managing emergency pain symptoms. The bolus administration provides almost instant onset with a short effect. Iv morphine is used when sc route is not tolerated. A popular application of this method is in Patient Controlled analgesia (PCA) where the opioid is delivered using a Hospiral infusion device. The patient can control the morphine intake and establish an acceptable level of analgesia6. Morphine is available in 50 and 100ml vials with strength 1mg/ml3. It is used post operatively especially after transplants surgeries and in the management of chronic pain of malignancy. Although this method is useful for the management of severe pain there are many drawbacks. First of all, most of the pumps are bulky and invasive. Their installation is complicated as they require mains attachment and the patient has limited mobility .There is a high risk of overdose therefore supervision is needed. Side effects such as hypotension and respiratory dep ression might develop while convulsions due to high dosage are likely to occur. fig3. PCA infusion device vs syringe driver Subcutaneous Subcutaneous method is an excellent alternative to oral administration method. It is safe and effective method which is widely used both in palliative care and severe pain management. It is used for patients that present conditions such as gastrointestinal disturbances including indigestion, palindromic motility and obstruction5. A prime characteristic of the sc method is the syringe driver. The driver employs simple syringes and bears a flow rate setting option usually ml/hr which enables accurate dosing over a specific period of time. Unlike PCA infusion devices, it is battery powered, so patients mobility is not an issue (fig 3). The risk of overdose is lower compared to iv route as there is a constant stable administration schedule6. There is no need for continuous strict supervision as the driver is easy to use. Often, the subcutaneous route can cause some skin site problems. Irritation might appear especially when there are high concentrations of morphine or when it is combined with other drugs which are not fully compatible3. This problem can be treated either by diluting the dose or by choosing alternative more compatible drugs. Also some patients might experience needle allergic reactions and action must be taken e.g introduce an alternative Teflon cannula. Patients might feel discomfort, especially those who lack of subcutaneous tissue16. Sometimes, possible leakage of subcutaneous site might be observed. Finally, the sc method is not recommended for palliative care patients with acute vascular conditions. Rectal This drug administration route is quite popular and is used for short term management of acute pain. Also in terminal cancer patients modified release morphine sulphate is administered via this route6. Although the absorption rate varies for each individual, it is estimated that around 300-330 mg of morphine are absorbed rectally every four hours, indicating a sufficient analgesic effect. Studies have shown that for the patients who have never taken morphine, rectal morphine is more effective than the oral administered one5. Rectal morphine is available in the form of suppositories at different strengths (10, 15, 20 and 30 mg) 3. Prescribers must specify both the strength and the morphine salt (morphine hydrochloride or morphine sulphate) that suppositories should contain. Evidence shows two deaths15 after consequent doses of rectal morphine should raise awareness and the need of monitoring the dosing regimen and frequency. Rectal route is not avoiding completely the first pass metabolism; therefore the bioavailability might be influenced. Non conventional routes Apart from the conventional drug routes mentioned before, it is necessary to look into other not so popular ones such as the transmucosal, transdermal, sublingual, intrathecal and nebulised routes of administration and try to compare them. Transdermal- Transmucosal Transdermal route is a relatively painless method. The drug absorption is rapidly. Patches are easy to use and not quite expensive. Although this method is usually tolerated from the patients skin irritation and rashes might appear. Transdermal morphine is not popular as there are questions about the efficacy of this method. However, fentanyl another opioid is the drug of choice. More specifically, fentanyl patches are more effective in managing chronic pain compared to modified release oral morphine9. Also, transdermal fentanyl causes reduced constipation and drowsiness14. Another administration route which fentanyl is again preferred than morphine is the transmucosal route by a buccal tablet. The main reason is that fentanyl is a lipophilic drug while morphine has a limited lipid solubility. More specifically, when fentanyl is placed in saliva it is 80% non-ionized  and it usually takes 20-30 minutes for the analgesic effect to reach its peak14. Note transmucosal morphine is also available but the bioavailability is low and analgesic effect is not significant. Sublingual route This route utilises soluble immediate release tablets and it is recommended to patients that have swallowing difficulties. Morphine enters into bloodstream via sublingual mucosa. Investigations have shown that the absorption of morphine is the same (and in a few cases faster) compared with the oral route7. Same studies have shown that there is no significant difference in the extent and duration of analgesic effect when is compared with oral administered morphine3. Nebulised morphine Nebulised morphine utilises the airways to deliver the drug into pulmonary circulation. It can be used for COPD patients in the management of acute thoracic pain11. Through this route, the effect of hepatic metabolism is avoided and a rapid absorption is noted. However, the plasma drug levels are unpredictable indicating the necessity of further clinical investigations. Studies showed that inhaled morphine was as effective as intravenous PCA morphine without causing strong sedating symptoms8. Although nebulised morphine is well accepted, patients who are not used to opioids might experience nausea and vomiting and in rare cases respiratory depression. Other common side effects include constipation and histamine induced broncho-constriction. Another drawback of nebulised morphine is the high cost of the equipment (nebuliser, injectable vials, etc). It is not a simple method and some patients might find it difficult to use the nebuliser. Studies show a wide range of bioavailability in subjects; relating the bioavailability of the drug with the patients ability to use the nebuliser in the right way10. Epidural/intrathecal Both epidural and intrathecal routes are used mostly in the hospital clinical environment. The bioavailability is high so spinal administered morphine can provide extended analgesic effect at lower doses (compared to the conventional drug routes)12. Although epidural and intrathecal morphine can relieve both acute and chronic pain; studies suggest that these routes should be used only for pain which cannot be controlled by the classic established methods13. For instance, these routes are effective in managing lower body pain. Note that only 1% of the daily iv dose must be used intrathecally and only 10% epidurally5. An implanted infusion pump may be used to deliver intrathecal morphine at a continuous rate. A disadvantage of these methods is the high risk of infection and overdose as the staff must be trained and careful when dosing for breakthrough pains. Studies have reported a number of patients experiencing side effects such as sedation, dyspnoea, nausea and vomiting after spinal administration12. Spinal morphine is not usually preferred while diamorphine is the drug of choice due to its high solubility manner. Conclusion As it was mentioned before, morphine can be administered via different routes. However, it is vital each time to consider the risk- benefit ratio for each method and choose the most effective and safe one. Moreover, before establishing unconventional methods such as nebulised and spinal morphine it is important to further investigate their suitability and ways to improve the drug delivery and minimise side effects. Finally, if it is necessary to choose other opiates (such as fentanyl and diamorphine), which might be more efficient than morphine.
